Same issue here. I have the 12 digit code but I keep getting the dreaded promo code "you entered is incorrect or is no longer valid". This is after already receiving an email from SE. And I check my Services List in Account Management to only see the Beta listed; no Early Access confirmation in sight. Do we just assume we have an EU code and that is why it is not working, as previous poster suggested? Therefore we are supposed to wait for another email from SE with a new, correct 12 digit PS3 code?
Some interest points: you can link your account on the na.finalfantasyxiv.com to your PSN account under My Profile -> Settings, though my PS3 code still doesn't work. Here are some pictures:
Linking a PSN Account to an SE account No Early Access in Services List I also noticed that, while FFXIV is registered on my na.finalfantasyxiv.com account, it does not actually show up on my secure.square-enix.com account.

EDIT: As to my earlier question of whether or not I had a EU instead of NA code, perhaps the address on the bottom of the email with my 12 digit code has evidence:
