since the extreme primal accessories are for "all classes" does that mean everyone can Need on them? would hate to go in there a healer only for healing earrings to drop only for some monk to need on em lol

No. That would be silly.
A Tank can't roll Need on "All DoW" DPS pieces, that much I know already.

A lot of mage gear is "all classes", but Need is still limited to the jobs that the gear is catered toward. So Need isn't based on just whether or not you can equip.
With all that being said, a lot of people make groups for this fights and try limit the looting to greed. This with the intention to invite people to come on their best/main class rather than the one they are trying to dress up.
