I'm resubbing tonight and my weapon/gear is garbageFollow

Where should I start? Last major patch I remember doing is Mhach.

Hey welcome to my world. I just resubbed too and my gear was hovering right around i200 after a free week of gametime a couple months ago.

I recommend starting with the main story and getting as far as you can in that. If you can unlock a set of dungeons great because they'll probably have gear upgrades for you. If you can't then you might be stuck like I was farming Lore tomestones for a while until you can buy your way into being cool again. Fortunately that's a pretty quick process. Weeping City of Mhach also no longer has a weekly lockout on loot so if you want to roll those particular dice for some upgrades, it's a good source too.

I recommend picking up your Wondrous Tails book before you do any instanced content though so you can be getting first-time bonuses for that and completing tasks as you go. The book is in Idyllshire from a little miqo'te girl on the west side of the building (it's a building now).

I actually don't know. I assume so. Even so though unless you're doing A5-9 savage the gear is unlikely to be worth the trouble when there are easier ways to get it.

I'd actually recommend doing Palace of the Dead.

One to get familiar with gameplay again, two, it's a quick, cheap way of getting seals, and three... as soon as... what tuesday? It will be dropping ilvl 255 weapons when you advance far enough with it. Four - no gear requirement as it has it's own progression system. So you are in no danger of being locked out of it.

Doing the story and unlocking your dungeons as well will help get you enough lore to jump you into 230 gear, which, with the weapon, will push you over the necessary ilvl to get through Alex Normal, which will drop 250 gear. After that, you're just back to the weekly tome grind.

Anyways, welcome back. Kick back and have some fun. Keep in mind this is likely the last ilvl hike until Stormblood, so no rush for things.

If you have the gil, level 250 stuff is available on the Market Board. On Ultros the HQ stuff would probably run about 15 to 20 million for the whole set, weapon included.

Seconding PotD. Level an alt job, get some hoard loot, and work towards a Kinna weapon.
