In beta, game runs pretty well on low settings, would like to make it better. With these specs what would make the biggest difference in performance to upgrade, OS + RAM or Graphics card?

Intel Core2 Quad @2.83Ghz
Vista 32bit
1gig DDR3 x2
Geforce 9600 GT 512MB

From general experience the game runs on your GPU more than anything else so I would personally upgrade that, too. RAM is nice too, but since FFXIV is so graphic-intensive I think you'll get a better performance boost out of the card first.

Actually, the game runs on both. What i mean is, if you get a kick **** graphics card, your CPU WILL bottleneck it. If i were you, i'd spring for a new GPU for now, and then plan on upgrading everything else when you have the time/money.
Edit: the Core2 line has proven to be sub par. You really want to jump up to i7 series if you want to improve game quality. I'm running a good graphics card, but my CPU (AMD 64 X2 6000+) is bottlenecking it. And my CPU is almost as good as yours is (benchmark wise).

Just upgrade your graphics card. I run a similiar setup with a geforce 260. So see what your powersupply can handle with connections etc. If you are going to buy, get something like a geforce 460 minimum. Don't waste your money on old stuff.

My setup
2.5 ghz intel core quad (4 cores)
2 sticks of 2 gb ram
1 geforce 260
Windows 7 64 bit
850 watt powersupply

I run a full-screen resolution of 1920x1080 with some goodies turned on.

a quad core 3.0 cpu is recommended. For AMD, they recommend a 6 core 3.0 CPU
