For every +1, add 5% to the starred stats.

If the Defense on item A is 65 and it is +1 the Defense of the item is really 68.
+2: 71.5 (not sure if rounded up or down, I assume up)
+3: 74.75

As you can see, an item with DEF 65 that is +3 has substantial gains.

Weapons work the same way, and only the starred stats gain the bonus.

You assume correctly. The formula ceils everything up, which means 4.2 defense would become 5 defense. (In such a case, there is no difference between a +1 and +3, which is the case for most rings/earrings)

NQ : 5 defense * 1.00 = 5.00 = 5
HQ1: 5 defense * 1.05 = 5.25 = 6 (+1 from NQ)
HQ2: 5 defense * 1.10 = 5.50 = 6 (+1 from NQ)
HQ3: 5 defense * 1.15 = 5.75 = 6 (+1 from NQ)
