Yes, you have to buy the game to play. Most retailers have it for $20 or so these days.

Yes, they are currently charging for subscriptions as of the beginning of this year. The monthly rate for one character is $10/month, lower if you commit to three or six months at a time.

Your specs are a little low for the game, however. Sometime next year they will be "relaunching" FFXIV in what's known as "version 2.0." It will be released for both the PC and PS3 and it's expected to have less stringent minimum requirements to play. It may be worth the wait.

your ram and cpu are fine. your gpu is comparable to a 1g 9500gt so itl do. i play with 2 gigs less ram and a dual 2.1 gpu and the game runs smooth with most settings turned up near 7 relatively well. sometimes i lag on and off in city's but other than that runs smooth. even in battle. give it a shot. if it isnt up to your requirements id suggest upgrading yer cpu and possibly harddrive to the new faster models of which i cant remember the name of right now.

First of all, that's the maximum; the minimum is several dollars lower than that. Second of all, that's a very low price compared to... well, really, almost every other subscription-based MMORPG in existence.

Judging from your questions and surprise, it sounds as though you've only experienced browser-based games. You need to learn a bit about the genre before you go around labelling things as "insane."

Even browser-based game can cost $6/month for subscription. What's insane though, is some indie title charging premium AAA subscription and then went bonkers a year later...
