That I know, nobody has really received a e-mail with instructions, but I have confirm with some friends that you should had received a e-mail with a invitation for beta phase 3.

service and options then
confirm account status then choose
Final Fantasy XIV it should show it as registered.
Here is the link to download the client. The Beta Phase 4 client is the same as the release. The client is about 128 MB, after installation the patch will be around 8GBs or more.

If you still remember your 1.0 username and password, that is what you need to log in. After the free 30 days probably will have to setup a payment option.
There was a time to move your 1.0 character to a difference server which already expired. SE has confirm that after a couple of months, they will open the option to move again like in FFXI. When you start the game and it will show your 1.0 character, you can click on the summary on the right side of the screen to change everything about them including race and name.
