I have checked and have been unable to find it. I was moving things around on the hot bar, and now the icon for sprint is gone. I have cycled through the alternate hot bars, and have found nothing. Any help would be appreciated.
Side Note: this is for PS3 user.

Sprint is a General Action. You should be able to grab a copy from the General menu, using whatever method you use to assign actions on the PS3.
Look under Actions and Traits > Actions and you will be able to find it and put it back on your hotbar.
Happens all the time to me. I just go grab it with square button > assign to hotbar from the icon forund in the genral actions menu under character.
Check out the little icon that looks like an open lock next to your cross-bar. You can click that to lock your cross bar so that doesn't happen again.
Just unlock it when you need to customize.
(There is supposed to be a feature in the menu where you can set it so you can't customize during battle, or at least you can toggle it off customization while weapon is drawn, but I'm not sure how that feature works.) Just locking and unlocking works for me.
